What Personal Information About Customers Does Elite Foot Health Ltd Collect?

If you have an in-person foot appointment with a Foot Health Practitioner through Elite Foot Health, we will require all patients to fill out a Patient Health Questionnaire, it is the patient’s responsibility to fully answer all questions and to update the Foot Health Practitioner of any changes to personal information.

Here are the types of personal information we collect:

            Information you give us: we receive and store any information you provide in relation to Elite Foot Health services. You can choose not to provide certain information. Elite Foot Health cannot be held liable for any loss, injury, or any consequence a patient or customer experiences due to non-disclosure, it is the patient or customers responsibility to make Elite Foot Health aware of anything which may affect the service you get from us.

            Automatic information: On the internet, like many websites, we use cookies and other unique identifiers and we obtain certain types of information when your web browser or device accesses Elite Foot Health Services. If you email, telephone, text, WhatsApp, or contact Elite Foot Health via social media or our website or in any other way, we may retain this information for a period of time in relation to UK law and GDPR.

Are Children and Vulnerable People Allowed to Use Elite Foot Health Services?

Elite Foot Health doesn't sell products for purchase by children.

Elite Foot Health’s Foot Health Practitioners may conduct an in-person foot appointment for a child, or a vulnerable person. In this instance an appropriate adult (a parent or carer with Parental Rights, or someone with Power of Attorney) is required to give permission both verbally and in writing on the Patient Health Record document by reading the document in its entirety and signing on the patients behalf. The person with Parental Rights, or an approved and agreed appropriate adult, or the person with Power of Attorney are required to remain with the patient throughout the entire duration of the in-person foot appointment and for any subsequent appointments. Elite Foot Health will also take in to account the wishes of the Patient and will always treat every patient with dignity and respect, regardless of age, or ability. If the patient has objections to the treatment, Elite Foot Health practitioners will not go forth with the foot appointment if the patient is portraying distress or does not give their permission, contrary to the permission of persons with Parental Rights or Power of Attorney; the patients consent must always be given in order to conduct an in-person foot appointment.

How Long Do We Keep Your Personal Information?

We keep your personal information in accordance to Data Protection Act 2018, and we fully comply with GDPR.

EU General Data Protection Regulation and UK Data Protection Laws - Legal Basis

The EU GDPR and UK data protection laws require a legal basis for our use of personal information. Our basis varies depending on the specific purpose for which we use personal information. We use:

            Performance of a contract when we provide you with products or services, or communicate with you about them. This includes when we use your personal information to take and handle orders, deliver products and services, and process payments.

Information You Give Us When You Use Elite Foot Health Services

You provide information to us when you:

            search for products or services in our stores;

            place an order through Elite Foot Health Services;

            provide information at point of purchase, or when you contact us

            communicate with us by phone, e-mail, or otherwise;

            upload images, videos or other files to Elite Foot Health Services;

            Provide a Customer Review;

As a result of those actions, you might supply us with such information as: your name; address and phone number; payment information; your age; your location information; your IP address; people, addresses and phone numbers listed in your Addresses ; e-mail addresses of your friends and other people; content of reviews and e-mails to us; images and video collected or stored in connection with Elite Foot Health Services, information and documents regarding identity and standing; corporate and financial information; credit history information; VAT numbers; and device log files and configurations.

Information From Other Sources

Examples of information we receive from other sources include:

            updated delivery and address information from our carriers or other third parties, which we use to correct our records and deliver your next purchase or communication more easily;

In-person Foot Appointments

A patient questionnaire will be required for all in-person appointments, including questions regarding medical history, age, address, GP surgery, agreement to disclosure, etc will be held in accordance with GDPR and data protection laws in the UK and EU. After each in-person appointment has been conducted, the Foot Health Practitioner will hand write notes about the appointment, which can be made available to the patient, or approved person acting on behalf of the patient, in accordance with the law; an administration charge may be payable up to the maximum of £25 per document. In the unlikely event that you suffer a minor injury, such as a cut, during treatment, any such incident will always be recorded at the time.

It is the patient’s responsibility to alert the Foot Health Practitioner of any medical conditions or medication changes.

If a patient dies Elite Foot Health will keep patient records for a maximum of 8 years, at which point; they will then be destroyed. If an adult patient discontinues their relationship with Elite Foot Health their Patient Health Record will be kept on file for a maximum of 8 years after the last data was recorded, at which point; they will then be destroyed confidentially. If the patient was 17 years of age on the date when the relationship with Elite Foot Health ceased, then the data held on these persons will be held until their 26th Birthday, at which point; they will then be destroyed confidentially. If the patient was under 17 years of age on the date when the relationship with Elite Foot Health ceased, then the data held on these persons will be held until their 25th Birthday, at which point; they will then be destroyed confidentially.
